Certifi ed Elite Installer Requirements:
Must have completed the Certifi ed Elite Installer Agreement
Must have at least one (1) Master Technician on staff with status valid and in good standing at all times
Must be involved with the project
Before Installation - Steps for Verifying a Cabling Installation:
1. Certifi ed Elite Installer must call their AUSVIGOR Regional Manager before the work is started or during the bidding process
2. Qualify the products going to be used for the cabling system
3. Complete this Lifetime Verifi ed Site Form
4. Get a current or updated horizontal and/or backbone schematic
5. Email both the form and schematic to AUSVIGOR for review
6. AUSVIGOR will issue a verifi cation number and email the Lifetime Verifi ed Site Form back to the Certifi ed Elite Installer

After Installation - Finishing the Process:
Within 30 days after completion of the cabling system installation, the Certifi ed Elite Installer must submit the following to AUSVIGOR:
1. Completed and signed Lifetime Verifi ed Site Form (hard copy)
2. Submit complete test documents, both in hard and soft copy, of the verifi ed installation according to the
most current TIA-568 specifi cations
3. Each link in the network must be fi eld tested in accordance with the TIA-568 industry standard enforced
at the time of purchase AND the installation network links must have passed all applicable TIA electrical
performance requirements. Minimum testing includes Attenuation, Near End Crosstalk, Far End Crosstalk,
Return Loss, PS NEXT, ELFEXT, PS ELFEXT, and ANEXT
4. A copy of the horizontal and/or backbone schematic;
5. A copy of each dated invoice for all AUSVIGOR products and horizontal premise cable used in the structured
cabling system; and purchased from an authorized AUSVIGOR distributor
6. Any other information as requested
Upon verifi cation of compliance of all the above, AUSVIGOR will issue a Proof of Warranty and attach to the Lifetime Performance Limited Warranty to the Certifi ed Elite Installer.
Making Modifi cation, Additions, Changes (MACs) to the Verifi ed Site:
In order to make a change, addition, or replacement to the original site of installation without voiding the warranty, the Certifi ed Elite Installer must submit to AUSVIGOR the
following information before the work is started:
1. A copy of the original Lifetime Verifi ed Site Form
2. Structured cabling site schematics refl ecting the changes to be made
3. A detailed list of the products to be used for the change, addition, or replacement;
4. An explanation of the reasons for the change, addition, or replacement; and;
5. Any other information requested
Upon completion of the MAC application process to AUSVIGOR’s satisfaction that the structured cabling system qualifi es for the Lifetime Performance Limited Warranty and is
properly documented, AUSVIGOR will acknowledge in writing to the Certifi ed Elite Installer they may then install the structured cabling system as documented.
Within 30 days following completion of the structured cabling system installation, the Certifi ed Elite Installer must re-submit documentation to AUSVIGOR. Refer to section, “After
Installation - Finishing the Process,” and repeat steps.
Certifi ed Elite Installer:
I certify that the information in this form is true and complete to the best of my knowledge and that any falsifi ed statements shall deem all warranties null and void. I
authorize investigation of all statements contained herein concerning this installation and any other pertinent information and release the company from liability for any
damage that may result from utilization of such information.
